Forms are extremely useful when creating in-place enactment characters.
Universal Tales has a number of them for players to use for personal use.
UT Character sheets (1 to 8) are available here and now (Sheet-1 interactive),
with more to become in the not so distant future.

The interactive forms are able to be saved in different names
for different characters and campaigns – changing the file title after downloading.
This can be handy for updating the characters and printing off clean sheets.
Note: The interaction is with copies downloaded. A PDF reader will be needed.
The first sheet is partially 'interactive' and may be printed blank or filled out as much as you wish.
More options will be available as forms are made more interactive.
Keep in mind, when using printed forms; filling out forms with pen is permanent. Pencils allows for updating the form as you go. However, some areas get changed more than others, wearing the paper. Adding a smidge of non-gloss cellophane tape allows for the area to be quickly erased and rewritten. Glossy cellophane tape does not take pencil markings well. When adding a cellophane tape, ensure nothing is already printed in the area.
The third sheet is a continuation of the second, allowing additional skills to be presented. Since Universal Tales has a considerable number of skills for the enrichment of characters, the level of character development is only limited by the player’s imagination.
The fourth sheet is dedicated to those who lean towards combat scenarios. If nothing else, most characters in most campaign environments find themselves in conflict at one time or another. When that happens, tracking blows and injuries usually becomes a matter of scratching figures on a piece of paper.
Granted, the amount of information on sheet-4 is more than many campaigns may consider, though many more who experienced the lack of information of other platforms will appreciate what is available with Universal Tales.
If sheet for is more than you would consider, ignore it – though the upper right corner does have a statistic spread that helps with the format of Universal Tales and may be handy for other platforms.
The Elaboration sheet is useful for outlining the characters family, goals, financial situation, occupation income, general health, as well as presenting the common actions and tasks with the information associated with it for ease of play.
To the uninitiated, Universal Tales appear complex and this sheet may seem overwhelming. However, the consistency throughout Universal Tales enables a unique realism to progression of outcomes.
As with all games, if sheet-6 is too much to comprehend, ignore it and move on. For those that understand the need, this sheet is well-suited to handle the stresses and strains characters go through during the events of the campaign.
As for the realism, way too many role-playing platforms allow characters to exceed reasonable motion while carrying considerable amount of materials. Anyone who has done any moderate hiking with any amount of gear nose house strenuous it can become. For that reason, this sheet is provided.
The Background sheet is quite handy for presenting the backstory and other aspects of the character’s life.
Separate sheets may be copied for personal use in order to outline separate aspects of the character; their childhood, military time (if any), family relations, and so forth.
This form can also be used for keeping track of what the character had done within the campaign, adding the date and a few notes to reference.

Wear and tear note: As most of us who played role-playing games around the table discovered, marking and erasing various parts of the form wears down that area. A little trick that is been handy over the years is to use cellophane tape just over portions that are going to be remarked over and over again.
Just remember, use the non-glossy cellophane tape – not any that are slick. You know the type. Pencils do not write well on the slick style.
Also remember, put the tape down before writing in that area. Putting cellophane tape on areas that you would like to keep permanent and unchanged is also handy, though one must make sure to lay down the tape on those before writing anything that would be changed later.
Of course – if the sheet gets worn, smudged, torn or otherwise undesired, one can always reprint the forms (or copy from fresh sheets).
Some people like to keep their character forms with in a small binder – even a large binder, if they have a great amount of information or number of characters. Using the cellophane tape where the holes will be punched reinforces that area, allowing the sheet to retain continuity within the binder.
Paper protectors are quite handy for forms – even if you decide not to use binders. Using paper protectors allows for water-soluble or dry markers to be used for temporary notation – a very handy thing for combat. In this particular situation, ensure that water-soluble or dry markers are used and not permanent markers.
Keeping forms in folders – if you choose not to use binders – allows for sorting and protection of the character sheets.
